Patients registered with the Nelson Medical Practice in Great Yarmouth are being asked to share their views on the practice to help shape future services.
NHS England, working on behalf of NHS Great Yarmouth and Waveney Clinical Commissioning Group, is asking patients to complete a short survey to help find a new provider for primary care services at the Pasteur Road surgery.
Available both online and in paper format, the anonymous survey asks for views about opening times and appointments, while also asking patients to rate the current services they receive and share their ideas for where further improvements could be made.
The results will be used to help NHS England put a new contract in place for services at the Nelson Medical Practice when the current contract, with East Coast Community Healthcare, comes to an end on 31 October.
Anyone who would like to share their views should complete the survey, which is available at www.smartsurvey.co.uk/s/nelson_haveyoursay/ by 25 May.
