A review of deaths at the James Paget University Hospital in Gorleston has found six out of a selected 406 deaths in 2017/18 have been identified as less than satisfactory.
The deaths were among 1,276 at the hospital in that period and following a review the six cases are subject to a full root cause analysis, with four of them being said to be potentially avoidable.
The figures in a mortality learning from deaths report were discussed by the hospital’s board of directors today.
The report says “learning themes” have been identified with actions agreed to rectify any issues, such as senior decision makers are in during out periods, an audit of admission documentation and death certificates being the focus area for a new medial examiner role.
Improvement plans also mention better clinical coding, a new health intelligence hub and ensuring learning is shared.
The board was asked to approve the report and its assurances.
